Headers and footers

Headers: used to display a text, image, or HTML based items at the top of the survey or report pages. Select new header, type in the name (used for identification in header list) in the first field and the content in the second. You can use HTML code in the header, but remember that the header is part of a web page, and must therefore not include any header tags (<html>, <body> etc). See HTML hints for more info.

Footers: Same as for headers, but will appear at the bottom of each survey or report page.
