Survey name is used in the admin screens only. It should be descriptive, to make it easier to find your survey later among a list of other surveys.

Title will appear at the top of the survey form (the survey respondent will see this).

The introduction will also appear on the survey form, below the title. Use this to give general instructions to the respondents (the persons filling out the survey form). This description is optional.
